Mission Community Hospital $1.55M Data Breach Settlement

1. Check whether you received a notification around November 21, 2023 from Deanco or a Deanco affiliate telling you your personal information may have been exposed in the May 1, 2023 breach. That notice is what makes you a class member.
2. If you lost money or spent time dealing with fraud because of this breach, pull together your supporting records now — bank statements, receipts, any correspondence. You'll need them to claim the up to $5,000 Documented Loss Payment.
3. File your claim online at https://forms.ksacms.com/efiling/fr/eform/deanco_healthcare_claimform/new?form-version=1.
4. Prefer paper? Mail a completed Claim Form postmarked by August 12, 2026 to: Concepcion et al. v. Deanco Healthcare, c/o Kroll Settlement Administration, P.O. Box 5324, New York, NY 10150-5324.
5. On the form, elect Medical Monitoring Services if you want two years of monitoring covered.
6. If you're an eligible California resident, the $100 California Statutory Payment is also selected through the same Claim Form.

What the form asks

• Claim Form — loss documentation — A description of your out-of-pocket losses tied to the breach, backed by receipts or records, to request reimbursement up to $5,000. (Pull from your bank statements, credit card records, or any correspondence about identity theft or fraud you dealt with after the incident.)
• Medical Monitoring Services election — Your opt-in for two years of Medical Monitoring Services. (It's a checkbox or selection on the Claim Form itself — nothing extra to track down.)

Watch out for

• The opt-out and objection deadlines are July 13, 2026 — a full month before the claim deadline. If you want to keep your right to sue separately, you need to act sooner.
• Filing a claim releases your right to sue Deanco and the Released Parties separately for the same incident.
• The Settlement Fund is fixed at $1,546,409.42. The more people who file, the smaller each person's share of the residual payment.
• Up to $235,400 of the fund goes to administrative costs before anything reaches class members.
• No money gets paid until the court approves the settlement at the Final Approval Hearing on September 9, 2026.
