If your personal information was exposed in the July 2025 Twin Cities Pain Clinic (TCPC) data breach, you can file a claim for up to $4,500 in documented losses or a flat payment of about $55, plus two years of free credit monitoring. Claims are due November 9, 2026.

Cash Payments are subject to an aggregate cap of $385,000.00 (reduced pro rata if exceeded).
