Valley Oaks Health Data Breach Settlement
Court case: In re: Valley Oaks Data Incident Litigation, Case No. 79D01-2403-PL-00032
If your private information was compromised in the June 2023 Valley Oaks Health data breach, you can file a claim for cash (up to $500 for documented ordinary losses, up to $5,000 for extraordinary identity-theft losses, up to $80 for lost time, or a flat $40 with no proof) plus two years of medical monitoring. Claims are due November 9, 2026.
Official eligibility wordingAll persons whose Private Information was potentially compromised as a result of the June 2023 Valley Oaks Health, Inc. Data Incident and who are included on the Settlement Class List. Excluded are the assigned Judge and court personnel/their families; Valley Oaks and its controlled/parent/subsidiary entities and affiliates; and anyone found guilty (or who pleads nolo contendere) of the criminal activity that caused the Incident.
