• Who may qualify You may qualify if Aetna's records show a denied claim or precertification request for a qualifying artificial insemination (ICI or IUI) or IVF billing code between September 1, 2017, and May 31, 2024, and you were in an Eligible LGBTQ+ Relationship at the time. Category A members had a documented denial and LGBTQ+ status on file; Categories B, C, and D cover people whose files are missing one of those pieces, including some later approved within 90 days or paid after the fact.
• Estimated payoutApproximately $10,000 Default Common Fund Amount payment (less if there are more than 200 Class Members, per an equal split of the $2,000,000 Common Fund), plus a $2,300 Default Dollars for Benefits Amount if applicable.

• What you needCategory A members do nothing to get the base payment. Categories B, C, D-A, and D-B must submit an Attestation Form and/or Claim Submission Form with supporting evidence through the dashboard at infertilityinsurancesettlement.com, or by mail, email, or fax, received by August 26, 2025. Anyone may also file optional Out-of-Pocket, Miscellaneous Harm, or Proof of Greater Covered Care submissions with documentation by the same date.
