What the form asks

• Cash Payment Option — Choose Cash Payment A (up to $4,000, receipts required) or Cash Payment B ($75, no documentation needed). Pick one — you can't claim both. (Review your options in the FAQ at https://alphabakingsettlement.com/faq/ before you start the form.)
• Credit Monitoring Enrollment — Indicate whether you want to enroll in two years of CyEx Financial Shield Complete. Have a valid email address ready — they'll use it to set up the service. (This option appears on the claim form itself.)

Documents you'll need

• Receipts, bank statements, or third-party proof of expenses caused by the data incident

Watch out for

• There's an aggregate cap of $1,050,000 on all cash benefits. If total claims push past that, every payment gets scaled down. The $75 flat payment could end up lower.
• The opt-out and objection deadline is also June 22, 2026 — same day as the claim deadline. Don't mix them up.
• Cash Payment A won't cover any expenses you've already been reimbursed for by a bank, insurer, or anyone else.
• Receipts and third-party documentation are required for Cash Payment A. A personal statement or affidavit alone won't qualify your claim.
• Court approval is still pending. The Final Approval Hearing is July 21, 2026 at 2:00 p.m. Payments won't go out until that's done and any appeals are resolved.
