- Confirmation that you received notice from Cadence Bank — Check the box or answer 'yes' to indicate that Cadence Bank notified you that your personal information may have been exposed in the MOVEit incident. (Look for the notice letter Cadence Bank mailed on or after September 15, 2023.)
- Benefit election (flat cash, ordinary losses, or extraordinary losses) — Select one: flat cash payment up to $100, ordinary-loss reimbursement up to $2,500, or extraordinary-loss reimbursement up to $10,000. (Your own records: receipts and expense logs for ordinary losses, or identity-theft reports for extraordinary losses.)
- Credit monitoring enrollment (if not already enrolled) — Indicate whether you want to enroll in two years of credit monitoring and identity theft protection services. (Check whether you already accepted Cadence Bank's original credit monitoring offer around September 2023.)
