• Who may qualify You are covered if you live in the United States and your private information was involved in the Coleman Professional Services data incident, including anyone Coleman mailed a direct notice to. The information at issue included names, dates of birth, Social Security numbers, driver's license or state ID numbers, financial account details, treatment or diagnosis information, patient ID numbers, and health insurance or payment information.
• Estimated payoutAll Class Members are eligible to submit claims for identity protection services and cash payments of roughly $70 (assuming a claim rate of 10% of the roughly 51,889-person Settlement Class), which will be pro rata increased or decreased based on the money still remaining in the Settlement Fund at the time of payment. In addition to these benefits, you are eligible to be reimbursed for any documented losses or expenses (up to $5,000 total) that you can demonstrate are traceable to the Data Incident.
