What the form asks

• LoginID — The LoginID printed on the notice they mailed you. (On the November 12, 2024 notice letter from Continental Cafe Holdings.)
• PIN — The PIN printed on the notice they mailed you. (On the November 12, 2024 notice letter from Continental Cafe Holdings.)
• Benefit election: Alternative Cash Payment or Documented Losses — Choose either the alternative cash payment of up to $50 or reimbursement for documented out-of-pocket losses up to $700. (The claim form itself.)
• Documentation for out-of-pocket losses — Third-party records showing losses tied to the data incident — credit card statements, bank statements, invoices, phone records, or receipts — from between October 18, 2024 and July 28, 2026. (Your own financial records.)

Documents you'll need

• November 12, 2024 notice letter from Continental Cafe Holdings (contains your LoginID and PIN) (required)
• Receipts, bank statements, invoices, or telephone records for documented out-of-pocket losses

Watch out for

• The $50 cash payment is subject to reduction based on how many people file — the final amount could be less.
• You can't take both the cash payment and the documented loss reimbursement. Pick one.
• Documented losses only count if they happened between October 18, 2024 and July 28, 2026.
• If you're bound by a Continental arbitration agreement from March 14, 2019 or July 18, 2023, you're not in the class.
• July 28, 2026 is the hard cutoff for both online submissions and postmarked mail.
