1. Confirm you received a notification letter from Cornerstone stating your Private Information was or may have been compromised in the Data Incident.
2. Determine which subclass you belong to: SSN Subclass (Social Security number potentially compromised) or general Settlement Class member.
3. Decide which benefit to claim: documented loss reimbursement, credit monitoring (SSN Subclass only), or a pro rata cash payment.
4. If claiming documented losses, gather non-self-prepared supporting documentation (receipts, bank statements, etc.) and prepare a description of how each loss is linked to the data incident.
5. Submit your claim online at cshealthcaresettlement.eagclaims.com, or download, print, complete and mail the PDF claim form to: Cornerstone Data Incident Settlement Administrator, PO Box 2271, Baton Rouge, LA 70821.
6. Keep a copy of your submitted claim and any documentation sent.

What the form asks

• — The notice ID printed on the settlement notice mailed to you by the settlement administrator. (On the notification letter / settlement notice you received from Cornerstone.)
• — Choose one: documented losses reimbursement, credit monitoring (SSN Subclass only), or pro rata cash payment. (Determined by your own situation and subclass membership.)
• — Receipts, bank statements, or other non-self-prepared records showing out-of-pocket expenses traceable to the data incident, plus a description of how each expense relates to the incident. (Your own financial records, bank, credit card statements.)
• — Choose either a paper check mailed to the address you provide, or a credit monitoring activation code (SSN Subclass). (Personal preference; select on the claim form.)

• This settlement is now past its claims deadline — filing is no longer possible unless a court extension was granted; verify the current status at the settlement website before attempting to file.
• Pro rata cash payments are not a fixed amount; the exact payment depends on the number of valid claims submitted and funds remaining after other costs are paid.
• Documented loss claims require non-self-prepared documentation — self-prepared notes or statements are not accepted.
• SSN Subclass members can claim extraordinary losses (such as identity theft costs), but only after exhausting all available credit monitoring and identity theft insurance.
