If you received a notification letter from Cornerstone Healthcare Group about the December 2023 data breach, you must submit a claim form to receive any benefit — no payment is automatic. SSN Subclass members can claim reimbursement for documented losses at a higher cap, credit monitoring, or a pro rata cash payment; all other class members may claim documented ordinary losses up to their cap or a pro rata cash payment.
