CVC Data Settlement

What the form asks

• Benefit election — Select the benefits you want: the estimated $75 cash payment, documented loss reimbursement up to $5,000, and/or two years of medical monitoring. (The options are listed on the claim form at the settlement website or the direct portal link.)
• Documentation for out-of-pocket losses — Attach third-party receipts, bank statements, or invoices that show losses tied to the data breach. Notes you wrote yourself aren't enough on their own. (Check your bank statements, credit card bills, and any invoices for services you bought to protect yourself after the breach.)

Documents you'll need

• Notice of the data breach received from CVC (optional but helpful)
• Receipts, bank statements, or invoices for out-of-pocket losses (required if claiming documented losses)

Watch out for

• July 1, 2026 is the hard cutoff. Miss it and you lose all benefits and give up your right to sue over this breach.
• Total individual benefits across the cash payment, loss reimbursement, and monitoring are capped at $5,000.
• The $75 cash payment is an estimate. The final amount depends on how many valid claims come in and what's left in the fund after costs.
• If you want out of the settlement entirely, you had to opt out by June 1, 2026. Opting out means no benefits but you keep your right to sue.
• The settlement still needed final court approval as of the notice date. Benefits aren't paid until the court signs off.
