Key facts

• Who may qualify You are covered if you live in the United States and Doxim, or one of Doxim's credit union clients, identified you and sent you a notice saying your private information was affected by the data incident around December 30, 2023. Directors, officers and employees of the defendants, the judge and court staff, and anyone who opts out are not included.
• Estimated payoutCash Payment A – Documented Losses: You may submit a Claim Form and provide reasonable documentation for fraud or identity theft losses that can be reasonably traced to the Data Incident for up to $5,000 per Settlement Class Member; OR Cash Payment B – Alternate Cash Payment: Instead of Cash Payment A, without providing documentation, you may submit a Claim Form to receive a Cash Payment in the estimated amount of $100; AND Credit Monitoring: In addition to Cash Payment A or Cash Payment B, you may also submit a Claim Form to receive one year of free Credit Monitoring. Your Cash Payment may be subject to a pro rata (a legal term meaning equal share) increase or decrease depending upon the total value of all Valid Claims.
