What the form asks
- Cash Payment Option — Choose Option A (documented losses, up to $2,500) or Option B (flat $65, no paperwork). Pick one — you can't claim both. (Details for both options are on the settlement FAQ at https://enduesoftwaredatasettlement.com/faq/)
- Email Address (for monitoring enrollment) — Your current email address so the administrator can send your CyEx Medical Shield Complete enrollment code. (Your own contact information.)
Documents you'll need
- Receipts, bank statements, or third-party proof of out-of-pocket losses
Watch out for
- The $65 Option B payments come from a fixed $260,000 pool. If enough people file, the per-person amount adjusts up or down so the total stays at $260,000.
- Option A won't cover losses that were already reimbursed by a bank, insurer, or any other third party.
- Receipts and bank statements work for Option A. Personal declarations and affidavits alone don't — you need third-party documentation.
- June 30, 2026 is the deadline for claims, opt-outs, and objections. All three cut off on the same day.
- The court's Final Approval Hearing is July 15, 2026. No money or monitoring goes out until after the judge signs off.
