1. Locate the postcard notice you received by U.S. Mail from Essen Medical — it contains a unique ID and PIN required to file online
2. Go to ehcsettlement.com/Login to file the online claim form, or download and print the PDF claim form at ehcsettlement.com/Content/Documents/Claim%20Form.pdf
3. Choose which benefit(s) to claim: the no-proof cash fund payment, the documented-losses reimbursement, or both
4. If claiming documented losses, gather and attach supporting documentation such as credit card statements, bank statements, invoices, telephone records, and receipts
5. If you did not receive a postcard or lost it, call the settlement administrator at 1-888-976-6880 before submitting
6. Submit your claim online or postmark the mailed form by the claim deadline
7. After submission, watch for an email from the administrator asking you to select your preferred payment method
