Hearthside Food Solutions child labor

1. Open the claim form at https://www.hearthsidefssettlement.com/il-ag-v-hearthside-claim-form/.
2. Fill in your name, current address, cell phone, and email in Section I.
3. Enter your date of birth in Section II, then upload a document that proves your age (birth certificate, passport, school ID, or similar).
4. In Section III, enter the dates you started and stopped working at the Hearthside Illinois facility, then upload proof of those employment dates (pay stubs, W-2, or personnel records).
5. Check the box in Section III if you still work at Hearthside — you can still file.
6. Read the attestation in Section IV, sign, and submit before November 20, 2026.
7. Not comfortable putting your SSN or ITIN on a web form? Call the administrator toll-free at (800) 410-0649 to provide it by phone instead.

What the form asks

• Name and Contact Information — Your full legal name, current mailing address, cell phone number, and personal email address.
• Date of Birth — Your date of birth in Month/Day/Year format. This is how the administrator confirms you were under 16 during part of your employment.
• Social Security Number or Individual Tax Identification Number — Your SSN or ITIN. This is used to issue your payment. (Your Social Security card or any prior tax return. If you'd rather not type it online, call (800) 410-0649 to give it by phone.)
• Employment Start Date and End Date — The date you first started working at the Hearthside Illinois facility and the date you stopped (or leave end date blank and check the current-employee box if you still work there).

• Proof of age (birth certificate, passport, school ID, or similar document showing date of birth) (required)
• Proof of employment history (pay stubs, W-2, personnel records showing dates worked at a Hearthside Illinois facility) (required)

Watch out for

• Immigration status has no bearing on your eligibility — anyone who qualifies on age and location can file.
• Payments won't arrive until Q1 2027 (January through March). There's nothing to do between filing and then.
• Your exact payment amount won't be known until all claims are in. It's a percentage of the available fund, weighted by how long you worked while underage.
• Your documents are kept confidential. Atticus shares them with the Illinois Department of Labor only as needed to process payments, and with no one else.
• Still working at Hearthside? You're still eligible. Check the box in Section III and file normally.
