1. Locate the settlement notice you received — it contains your unique ID and PIN required to file.
2. Decide which benefit you want: reimbursement of documented monetary losses, the alternative pro rata cash payment, or (if applicable) the additional payment for compromised clinical photographs.
3. File your claim online at lipsgsettlement.com/Login or download and print the PDF claim form and mail it to the settlement administrator.
4. If claiming documented losses, gather and attach supporting documentation such as receipts, bank or credit card statements, police reports, or other proof of fraud or identity theft.
5. If mailing, send your completed claim form to: Long Island Cybersecurity Incident Settlement Administrator, Attn: Claims, P.O. Box 6810, Portland, OR 97228-6810.
6. Select your preferred payment method: digital payment or paper check.
