M.A. Ford $425,000 Data Breach Class Action Settlement

1. Locate the unique ID and PIN from the settlement notice M.A. Ford mailed you — you will need them to log in to the online claim form.
2. If you cannot find your ID and PIN, email info@MAFordSettlement.com with your full name and mailing address to request them.
3. Go to the online claim form at mafordsettlement.com/form/claim, or download the PDF form from mafordsettlement.com/documents/ to mail or email instead.
4. Select every benefit type you are claiming: out-of-pocket losses, identity theft or fraud losses, attested time, pro rata cash payment, and/or credit monitoring.
5. For out-of-pocket or identity theft losses, attach receipts, bank statements, or other supporting documents showing the expense was related to the data breach.
6. For an attested time claim, briefly describe the tasks you performed and the time you spent on each.
7. Submit the completed form before the claim deadline. If mailing, send to: M.A. Ford Data Security Incident Settlement, c/o Settlement Administrator, PO Box 25226, Santa Ana, CA 92799-9958.

What the form asks

Watch out for

• Notes or self-prepared statements alone are not sufficient to support an out-of-pocket or identity theft loss claim — you must attach third-party documents such as receipts or bank statements.
• The combined total for out-of-pocket losses and attested time is capped per person; claiming multiple categories does not guarantee the full maximum.
• If total claims exceed the remaining fund, the settlement administrator will reduce all payments on a pro rata basis.
• Attorneys' fees and settlement administration costs are paid from the fund before class members receive anything.
• Payments will not be distributed until after the court grants final approval and resolves any appeals, so there may be a significant wait after filing.
