1. Locate your unique ID and PIN from the settlement notice mailed to you — online submissions require these.
2. Decide which benefit type to claim: ordinary losses (documented expenses), extraordinary losses (identity theft/fraud losses), lost time, the alternative cash payment, or credit monitoring.
3. File online at mrdatasettlement.com/form/claim, or download the PDF claim form and mail or email it to the settlement administrator.
4. If mailing, send to: M&R Data Incident Settlement c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799-9958. If emailing, send to info@MRDataSettlement.com.
5. If claiming ordinary or extraordinary losses, attach supporting documentation (receipts, bank statements, police reports, etc.).
6. If claiming lost time, include a brief written description of the time you spent dealing with the breach.
7. Submit your claim before the claim deadline has passed.
