How do I file a claim for this settlement?

   Locate your settlement notice from Nissan North America, which contains your unique class member ID

   Choose your benefit option: alternative cash payment, ordinary documented losses claim, extraordinary losses claim, or credit monitoring

   File your claim online at the official claim URL using your class member ID, or print and mail the PDF claim form to the settlement administrator

   If claiming ordinary documented losses, attach supporting documentation such as receipts, bills, or statements showing the data breach caused the losses

   If claiming extraordinary losses, attach third-party documentation showing losses due to fraud or identity theft, plus a police report or insurance claim
