- Choose your payment option on the form: Cash Payment A (documented losses up to the stated maximum, with supporting receipts/statements) or Cash Payment B (flat alternative payment, no documentation required).
- Optionally elect one year of three-bureau credit monitoring on the same Claim Form.
- If mailing, send the completed and signed form with all supporting documents to: PS Logistics Settlement Administrator, P.O. Box 301134, Los Angeles, CA 90030-1134.
- After submitting, retain a copy of your claim and any confirmation for your records.

- You must have received a notice letter from PS Logistics — only individuals who were sent that letter are in the Settlement Class.
- Losses claimed under Cash Payment A must not have already been reimbursed by another source; reimbursed losses are explicitly ineligible.
