You must submit a Claim Form to receive any cash payment or credit monitoring benefits — doing nothing means you get nothing. You can choose between a flat alternative cash payment or a higher documented-losses payment if you have receipts, plus optional credit monitoring. This claim form asks for 4 fields and 1 document.

  1. Locate the notice letter you received from PS Logistics — it contains your unique Claim ID and PIN needed to file online.
  2. Go to www.transportationbreachsettlement.com and submit the online Claim Form using your Claim ID and PIN, OR download the PDF Claim Form from that site and mail it to the Settlement Administrator.
  3. Choose your payment option on the form: Cash Payment A (documented losses up to the stated maximum, with supporting receipts/statements) or Cash Payment B (flat alternative payment, no documentation required).
  4. Optionally elect one year of three-bureau credit monitoring on the same Claim Form.
  5. If mailing, send the completed and signed form with all supporting documents to: PS Logistics Settlement Administrator, P.O. Box 301134, Los Angeles, CA 90030-1134.
  6. After submitting, retain a copy of your claim and any confirmation for your records.
