RFK Racing Data Breach Class Action Settlement

1. Locate the unique ID and PIN from the settlement notice RFK Racing mailed you; you will need these to log in to the online claim form
2. Go to https://rfkracingdatasettlement.com/form/claim and log in with your ID and PIN
3. Select Payment Option A (documented losses reimbursement) or Payment Option B (alternate cash payment, no proof required)
4. If choosing Option A, attach receipts or other third-party documentation for each out-of-pocket expense you are claiming
5. Select your preferred payout method (PayPal, Venmo, Zelle, virtual prepaid card, or physical check)
6. Submit the completed form online before the claim deadline, or print and mail it postmarked by the same deadline to: RFK Racing LLC Data Incident Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799-9958
7. If you cannot find your ID and PIN, email info@RFKRacingDataSettlement.com with your full name and mailing address to request them

What the form asks

• — The PIN printed on the settlement notice mailed to you (The settlement notice you received from RFK Racing)
• — Choose Option A (documented losses, up to $2,500) or Option B (alternate flat cash payment, no documentation required) (Determine based on whether you have receipts for out-of-pocket expenses related to the breach)
• — Receipts, invoices, or other third-party records for each expense claimed (Your own financial records, bank statements, credit monitoring service invoices, or correspondence with institutions)
• — Select one: PayPal, Venmo, Zelle, virtual prepaid card, or physical check (Your own preference; provide the relevant account details for digital options)

Watch out for

• Personal certifications, declarations, or affidavits alone are not accepted as sufficient documentation for Option A — you need third-party receipts or invoices
• You cannot claim reimbursement under Option A for expenses already covered by a third party
• If you cannot locate your notice ID and PIN, contact the administrator before the deadline so you have time to complete the form
• Opting out or objecting has a separate earlier deadline — missing it means you cannot exclude yourself from the settlement
