What the form asks

• Settling defendant / medication type — Select whichever settlement(s) cover you: Hetero Valsartan, Aurobindo Irbesartan, and/or Vivimed Losartan. You can claim more than one if you bought multiple medications. (Check your prescription records or pharmacy packaging to confirm which drug and manufacturer you had.)
• NDC codes (Vivimed Losartan claimants only) — Enter the NDC code(s) from your losartan packaging that match one of the eight impacted codes listed in the settlement. (On the pill bottle label or pharmacy receipt — it's an 11-digit number in the format XXXXX-XXX-XX.)
• Purchase dates and quantities — The number of 30-day supplies you purchased during the qualifying period. For Hetero Valsartan, up to 3 supplies at $40 each, capped at $120. (Pharmacy fill history or the explanation-of-benefits statements from your insurance.)

Documents you'll need

• Pharmacy receipt or prescription record showing purchase of the qualifying medication

Watch out for

• The claim deadline was June 2, 2026. Contact the administrator now to find out if late claims are still being considered.
• The Fairness Hearing is set for June 30, 2026, so the settlement isn't final yet.
• Each medication is a separate settlement with its own eligibility rules. If you bought more than one qualifying drug, file under each.
• For Vivimed Losartan, eligibility is tied to those eight specific NDC codes. Generic or other-brand losartan doesn't qualify.
• If you skip filing, you won't get paid and you give up the right to sue Vivimed over these claims.
• If you're an insurer or pharmacy benefit manager, there's a separate claims process at tpp.sartanmedicationsettlement.com.
