1. Locate the postcard or email notification you received from Phil Smith Management Inc. — it contains your class member ID, login, and password needed to file
2. Decide which benefit to claim: documented out-of-pocket expenses (requires receipts/records), an alternative cash payment (no documentation needed), or three years of credit monitoring
3. Submit your claim online at https://www.claimsettlementportal.com/surveys/PSMGMT/home.aspx using the login and password from your postcard or email
4. Alternatively, download the PDF claim form, print and complete it, then mail it postmarked by the claim deadline to: Phil Smith Management Settlement, c/o RG/2 Claims Administration LLC, P.O. Box 59479, Philadelphia, PA 19102-9479
5. If you no longer have your postcard or cannot locate your login credentials, call 866-742-4955 or email info@rg2claims.com to request that the settlement administrator provide your information
6. If claiming out-of-pocket expenses, attach documentation such as receipts, bills, or other records showing the loss and connecting it to the breach

• The claim deadline has passed — the filing deadline was April 27, 2026, so new claims cannot be submitted at this time
• You must have received a written notification from Phil Smith Automotive Group to be eligible; you cannot self-certify eligibility without that notification
