• — Choose one: documented out-of-pocket expenses, alternative cash payment, or credit monitoring (Decision is yours — review your eligible expenses before selecting)
• — Attach receipts, bills, or other records showing each loss and its connection to the breach (Your personal financial records, bank statements, phone bills, credit monitoring invoices, or gas receipts)
• — Select electronic transfer or paper check (Option presented during online filing)

Watch out for

• The claim deadline has passed — the filing deadline was April 27, 2026, so new claims cannot be submitted at this time
• You must have received a written notification from Phil Smith Automotive Group to be eligible; you cannot self-certify eligibility without that notification
• Out-of-pocket expense claims require documentation; selecting that option without attaching receipts or bills will likely result in denial of that portion
• Eligible out-of-pocket expenses are limited to specific categories — only unreimbursed bank fees, phone charges, data charges, postage, gasoline for local travel, and fees for credit reports or identity theft insurance purchased within the defined period qualify
• The alternative cash payment is pro rata and will be determined by the number of valid claims filed and remaining funds after deducting credit monitoring costs — the exact amount is not fixed
