3. File online at uldatasettlement.com/form/claim using your unique ID and PIN, OR download the PDF claim form and mail or email it to the settlement administrator.
4. If claiming expense reimbursement, attach supporting documentation such as receipts, invoices, bank or credit card statements showing unreimbursed fees or fraudulent charges.
5. Optionally elect to receive three years of credit monitoring with identity theft insurance — this can be selected alongside your payment option.
6. Submit your claim before the claim submission deadline listed in your notice.

Watch out for

• You must have received a mailed notice from Universal Lenders to be eligible — self-identified class members who did not receive a notice may not qualify.
