1. Go to the online claim form at www.BigBadCoughSettlement.com/submit-claim or download and print the PDF claim form
2. Fill in your name, address, and contact details
3. Check the boxes that confirm you saw the Big Bad Cough ad, received BOOSTRIX in New York or while living in New York, and received it to protect someone else from pertussis
4. Provide the approximate date and location of vaccination, the name of the provider or pharmacy, and the name(s) of the person(s) you sought to protect
5. Attach qualifying proof of vaccination if you have it (optional, but increases payout)
6. Select your preferred payment method: Venmo, Zelle, virtual prepaid card, or paper check
7. Sign and date the form, then submit online or mail to: Big Bad Cough Settlement, Attn: Claim Forms, 1650 Arch St., Suite 2210, Philadelphia, PA 19103

What the form asks

• — The city, state, or facility where you received the BOOSTRIX vaccine (Medical records or pharmacy receipts)
• — The name of the health care provider or pharmacy that gave you the vaccine (Medical records, vaccine card, or pharmacy receipt)
• — The name(s) of the person or people you were trying to protect from pertussis when you got vaccinated (Your personal recollection)
• — Choose one: Venmo, Zelle, virtual prepaid card, or paper check (Your preference)
• — Your signature (or electronic equivalent) and the date you are signing (Sign yourself)

Watch out for

• The claim deadline has passed — verify with the settlement administrator whether late claims are accepted
• Women who received BOOSTRIX while pregnant are excluded from the class and cannot claim
• You must have received BOOSTRIX specifically to protect one or more other individuals, not just for your own protection
• Without proof of vaccination you receive a lower payment; attach documentation if available to maximize your payout
• Proof documents must have been created before Feb. 10, 2025 to qualify
