If your UIM benefits were reduced by a Schmick offset, you must submit a claim form to receive payment. If you only paid premiums for UM/UIM coverage without having a claim offset, you do not need to file — the settlement administrator will automatically process your partial premium refund. This claim form asks for 3 fields and 1 document.
