What the form asks

• — The claim ID printed on the postcard you received by mail from the settlement administrator. (On the postcard mailed to you by the settlement administrator.)
• — The identification number printed alongside the claim ID on your mailed postcard. (On the same postcard as the claim ID.)
• — The NCAA Division I school(s) where you served as a volunteer coach during the class period. (Auto-populated from administrator records; correct it if wrong by completing all sections manually.)
• — The sport(s) in which you served as a volunteer coach (must not be baseball, FBS football, or men's/women's basketball). (Your appointment letter, team roster, or staff listing.)
• — The start and end dates of your volunteer coaching service, within March 17, 2019 – June 30, 2023. (Appointment letters, offer or onboarding emails, signed volunteer coach agreements, or team rosters.)
• — Select either direct deposit (ACH) with your bank account details, or request a mailed check. (Your bank account information for ACH, or your current mailing address for check.)
• — Your legal signature certifying the accuracy of the claim. (Sign at the designated signature line on the form.)

Watch out for

• Coaches in baseball are excluded from the settlement class and cannot file a claim.
• Coaches in FBS football and men's/women's basketball are also excluded from the class.
• Taking no action means you receive no payment and you forfeit your right to pursue independent legal action.
• If the administrator's pre-populated school data is wrong, you must affirmatively dispute it by completing all sections and submitting supporting materials — silence is treated as agreement.
• Payment will not be issued until after the court resolves any appeals and grants final approval of the settlement.
• Your actual payment depends on the number of valid claims submitted and the wages of the lowest-paid coach on your team — the estimated average is not guaranteed.
